Gardasil eller vaccinkandidat V503? Nytt HPV vaccin på G!


Gardasils uppföljare vaccinkandidat V503 har tillverkats och testats idet fördolda. Jag fann informationen när jag tittade in på www.dagensmedicin.se :s hemsida.

Jag tror säkert att de som valt att vänta med Gardasil, tills dess att man vet lite mer, känner sig ganska glada nu. Ytterligare ett alternativ dyker snart upp på vaccinhimlen med ny nytta och nya risker att sätta sig in i.
Vad var det som gjorde att Merck så snart började fokusera på att ta fram ett nytt vaccin med lite fler hpv-strängar i? Varför gjorde man inte det ifrån allra första början? Handlar detta om att bryta mark i närkampen med GSK?

Jag kan förstå varför Merck förutspår att V503 kommer att röra runt i grytan ordentligt.


Gardasil ges i skolorna trots att man vet att det ligger ett nytt vaccin
i pipelinen..... flickor tackar nej trots att vaccinet är gratis......

Merck's improved Gardasil could cause a stir.


As the global debate about the cost effectiveness – and in some places ethics – of human papilloma virus (HPV) vaccines rumbles on, Merck & Co has been quietly working away on a 21,000 patient phase III program for an improved version of its blockbuster vaccine  Gardasil.

Relatively little is known or has been written about the new candidate dubbed V503, yet it currently ranks as Merck’s second most valuable pipeline product at just under $1bn. As a 9-valent vaccine  V503 has the potential to protect girls and young women from cervical cancer caused by a wider range of types of HPV than the quadrivalent  Gardasil. Results from a 15,000-patient trial of V503 compared to  Gardasil should be released within a few months when HPV vaccines will be in the spotlight once more. Positive results would provide  Merck with more ammunition in the still two-way battle with  GlaxoSmithKline’s Cervarix.



Reenergising
Gardasil entered the market first in 2006 and made spectacular use of its first-mover advantage – the vaccine raked in global sales over $2bn in 2007, its first full year on the market.

However, revenues peaked in 2008 at $2.41bn, falling back to $1.36bn last year and they are currently predicted to slowly decline over the next six years. As it stands vaccination with Gardasil requires three injections over seven months but there is no requirement for booster jabs, meaning that use of the vaccine in teenage girls quickly reached saturation point in many countries.

Meanwhile Merck’s attempts to expand use of the vaccine into older women failed to gain regulatory approval and sanctioning of the product to protect boys and men from genital warts has been a marginal opportunity (Boys prove to be of little value to Merck, September 10, 2009).

In contrast Cervarix’s evolution has been more modest, entering the European market in 2007 but only gaining FDA approval in late 2009, almost three years after Gardasil. However, the vaccine has had some notable reimbursement successes in some countries, often pitched at a slightly lower price than Gardasil, and revenues are forecast to grow from $374m last year to $892m by 2016.

The expectation that Cervarix offers protection for longer – estimated at six years compared to four years with  Gardasil – has undoubtedly also helped Glaxo’s vaccine gain market share.



Greater protection
Both vaccines protect against two HPV types (16 and 18) that are estimated to cause 70% of cervical cancers. Cervarix is a bivalent vaccine against those two strains alone, but  Gardasil also protects against the two further types (6 and 11) which cause 90% of genital warts.

V503 is designed to protect against a further five types of HPV (31, 33, 45, 52 and 58) that also cause cervical cancer, offering much more comprehensive protection, particularly in more developing parts of the world.

In this instance there are some similarities to Wyeth and now  Pfizer’s successful extension of its pneumococcal vaccine franchise, gaining approval and then gradually switching vaccination regimens to its 13-valent  Prevnar from the original 7-valent version (

As such, the significant potential in V503 is clear if it can generate a positive set of results from seven phase III trials which have either completed or nearing completion (see table below).

The key trial appears to be V503-001 which enrolled 14,600 young women aged 16-26 years, which uses Gardasil as the active comparator. The study completed in August and given that  Merck has guided to filing an NDA for the vaccine next year, results should be forthcoming in the not too distant future.



Under the radar
Despite the potential for V503 to extend a valuable franchise like  Gardasil it is slightly surprising that the product has only ever been mentioned in brief dispatches by  Merck, which might also explain why it seems to be flying below the radar of most analysts.

Current consensus from EvaluatePharma put sales of  V503 at $300m by 2016, but this is heavily risk adjusted given that most analysts have not included the vaccine in their models.

Leerink Swann is one broker predicting V503 to be a very important product for  Merck assuming it enters the market in 2013 - analysts at the bank are expecting a dramatic switch from  Gardasil to  V503 with 2016 sales of  Gardasil to fall to just $195m as  V503 rises to $1.35bn.

With HPV vaccines still attracting much media interest and V503 flying below the radar, positive pivotal results from  V503 could cause quite a stir in more ways than one.

Det finns ett antal voffare “barkers”, lobbyister, som roar sig med att terrorisera dem som skriver om vaccin, dess nytta och risker. De kallar dessa “bloggare” eller skribenter för vaccinmotståndare, foliehattar etc. och anklagar dem bland annat för att orsaka död hos unga flickor, (avser här: Gardasil).
De begär vetenskaplig information för att skjuta varje påstående i sank, de sitter som skrikande fågelungar med munnen öppen. Minsta lilla som kommer in får dem att spy galla, en kritisk djupt kränkande sådan. De sprider rädsla och olust.

Men små fågelpojkar..... varför har ni inte berättat om vaccinkandidat V503? Vem jobbar ni för egentligen? En bra man reder sig själv, hämtar eget underlag och använder det som diskussionsunderlag. Fågelungar skall sitta kvar i sitt bo, invänta mat och kunskap, innan de flyger ut i världen.

Flickor, kvinnor är fullständigt kompetenta att ta sina egna beslut. Vi vill inte bli styrda av livmodersfixerade pojkar eller män. Ni bär inte vår smärta, ni bär inte våra sjukdomar och ni har inte rätt att göra valet åt oss.
Uttryck gärna era känslor men tala inte om för oss vad vi vill, vad vi behöver, vad vi får lov att prata om, vad som är bäst för oss eller vad vi skall välja.

Många av oss flickor och kvinnor vill göra våra val tillsammans med familjen efter genomgång av fakta och samtal med expertis.
